GIPSIL SERVICE IN WAKEFIELD
GIPSIL CAMHS TRANSITIONS WELLBEING OUTREACH SERVICE
(This is not a SEND specialist service, however we do support young people with neurodivergence) 
Criteria 
- Aged 16-25 years
- CAMHS experienced
- Have an ongoing wellbeing need
- Seeking support
Support
- Up to 18 sessions of 1:1 wellbeing support in the community.
- Weekly sessions for approximately 1hr (this can be less and a gradual build up if needed).
- Meeting where they feel most comfortable. (We tend to meet at cafés, libraries, and local parks, but we can make home visits).
- Blended approach if needed i.e. phone, video call and face-to-face.
- Holistic support, creating a support plan together with realistic goals.
- Wellbeing support examples - Safe space to talk, coping strategies, graded exposure, safety planning, risk management.
- Additional support examples - Advocacy at appointments, benefit support and applications, engagement in community activities, travel training and onward referrals for specialist services supporting the transition.
